Our team provides comprehensive functional assessments to help understand the strengths and needs of each individual.

Functional Assessments

We provide official NDIS reports and referrals to support planning and reviews. We complete comprehensive assessments for the individual so that they can obtain the necessary funding and support they require.

Our programs are delivered at Glenwood Farm and can be delivered in your home (by arrangement).

Funding Options

  • Funding options: Plan-Managed or Self-Managed.

  • We also offer a fee-for-service option for individuals who are not eligible for NDIS funding.
